Another way around it might be to enable logging to media/hdd, which might write lines to the logfile often enough to prevent spindown. Haven't tested it though.

The V2 has a setting to disable HDD spindown. Failing that, try and find out which USB controller chip your HDD uses and look for a firmware flashing tool to disable its automatic spin down (this was the solution that worked for me).

What makes you think that? EPGRefresh just visits services periodically. Does it actually interact with Autotimers in any other way (other than having a setting to run an autotimers scan after it runs)? I could be mistaken, but doesn't EPGRefresh rely on all the same mechanisms that fetch EPG info ...

I've noticed this issue as well. For me it was on network 10 with recordings of MotoGP. I will try Grumpy Geoff's solution and report back if it works.
I thought AutoTimer checks the event description for uniqueness, and since they have the same description it should not add the duplicate anyway.

A bug in the patch installer has been corrected where if one of the payload files fails MD5 verification, instead of presenting an error message to the user, the plugin would simply crash. I'm currently observing gaps in the guide data for ABC, but not on my TVs internal tuner EPG. This is contrary to the previous observation where both showed gaps on SBS channels. Perhaps there is more than one issue at play here.

There isn't much point in trying to get your head around the convoluted epgcache code if it isn't the source of the problem. I suspect it is the source of the problem, because when the issue occurs, querying the epg from Python via eEPGCache::lookupEvent (such as when printing self.list in EpgList....
